CCS Cases: Where Step 3 Is Won or Lost
The Computer-Based Case Simulations are unique to USMLE Step 3 — and they are the component most candidates are least prepared for. CCS cases do not test recall. They test your real-time clinical judgment: can you manage a patient from initial presentation through diagnosis, treatment, and follow-up efficiently and accurately using the Primum software interface?
CCS performance contributes significantly to your overall score and can determine your pass or fail outcome. Each case ends with an appropriate disposition. Knowing when to discharge, admit, escalate to ICU, or arrange follow-up is part of your clinical score and often determines the final mark on each simulation.

Step 3 Exam Content and Eligibility Requirements
Step 3 covers a broad range of clinical medicine across all major organ systems and care settings. Unlike Steps 1 and 2, the emphasis shifts from diagnosis to management, with a heavy focus on patient management decisions (45–55% of questions), appropriate use of diagnostics, and systems-based practice. You must have passed both Step 1 and Step 2 CK and hold an MD or DO degree from an accredited institution. IMGs must have obtained ECFMG certification. All applicants register through the Federation of State Medical Boards (FSMB) at fsmb.org.

When to Take Step 3
The USMLE recommends taking Step 3 after completing at least one year of postgraduate training in a US-accredited GME program. Many residents take it in their first or second year. IMGs should take it as early as eligibly possible. Busiest testing windows are May through July and November through December — schedule early.

Step 3 is reported as a three-digit score on a scale of 1–300. The minimum passing score is 200. The average first-time taker scores approximately 227. Results are typically released on Wednesdays, 3–4 weeks after your exam date.

Retake Policy
You may retake Step 3 up to a maximum of six total attempts, including incomplete attempts. State medical boards may have additional requirements. With only six lifetime attempts, every retake must be preceded by targeted tutoring to address the specific gaps that caused the previous failure.

Total question count and overall exam time are unchanged — but pacing per block has changed significantly. Candidates used to the previous 6-block structure must re-calibrate their timing strategy.

All USMLE Step 3 registration, score delivery, and support is now managed directly through the FSMB. IMGs previously registered via ECFMG should use the centralized USMLE portal. ECFMG still handles certification requirements but no longer manages exam registration directly.

The Step 3 minimum passing score of 200 remains unchanged. The CCS component on Day 2 — 13–14 interactive patient simulations using the Primum software — also remains unchanged in format, scoring weight, and case types. All content blueprints are stable.

Who is eligible to take USMLE Step 3?
To be eligible for Step 3, you must have passed both Step 1 and Step 2 CK. You must also hold an MD or DO degree from a US or Canadian accredited school, or from an institution listed in the World Directory of Medical Schools as meeting ECFMG eligibility requirements. IMGs must have obtained ECFMG certification. All applicants register through the Federation of State Medical Boards (FSMB) at fsmb.org.
What changed about the Step 3 format in the recent update?
Effective March 10, 2026, the block structure for both exam days was updated. Day 1 (FIP) now uses 12 blocks of 18–20 questions at 30 minutes per block, replacing the previous 6 times 60-minute format. Day 2 (ACM) now uses 9 MCQ blocks of 20 questions at 30 minutes per block. The total number of questions, overall exam duration, passing standard, and CCS format are all unchanged. Our tutors are fully updated on the new timing strategy.
What are the CCS cases and how do they affect my score?
Computer-Based Case Simulations (CCS) are interactive patient management scenarios on Day 2. You will encounter 13–14 cases across different clinical settings including outpatient, ED, inpatient, ICU, and patient home. Each case requires you to order tests, initiate treatments, advance simulated time, and adjust management as the patient's condition evolves. CCS performance contributes meaningfully to your overall score and can determine pass or fail outcomes. Candidates who do not practice with the Primum software beforehand are at a documented disadvantage.
Does MedSmarter offer Step 3 tutoring for IMGs?

What is the passing score for USMLE Step 3?
The current passing score for USMLE Step 3 is 200 on the three-digit scale. The average score for first-time takers is approximately 227. Unlike Step 2 CK, Step 3 scores are less commonly used as a residency differentiator — but passing is required for full, unrestricted medical licensure. Results are typically available on Wednesdays, 3–4 weeks after your exam.
When is the best time to take USMLE Step 3?
The USMLE recommends taking Step 3 after at least one year of postgraduate training in a US-accredited graduate medical education program.
