- Letter of Good Standing
- Health Records (immunizations, physical exam, PPD, flu vaccine)
- Criminal Background Check
- 10 Panel Drug Screen
- Malpractice Insurance
- USMLE Board Exam Report (if available)
- Copy of Photo ID or Passport
